You may need a New or First Passport if:

• You have never had a passport before.
• Have lost a valid passport.
• Passport is currently expired & was issued more than
  15 years ago.
• Passport is currently expired & was issued prior to your
  16th birthday or you are under age 16.
• Your passport is damaged or mutilated.
• Your name has changed since your passport was issued and
  you do not have a legal document formally changing your name.

Requirements for a New or First Passport are

For the Passport Office (Required Documentation's are):

• 2 (two) passport sized photos.
• Proof of US Citizenship - Original/Certified Birth Certificate or Naturalization Certificate or a previous US Passport.
• Proof of Identity - Valid Driver's License (not temporary or learner's license) or Previous US passport or Certificate of Naturalization or government employee photo ID as Proof of Photo Identity.
• Passport Application Form DS-11 completed Online.
  - Note: In the Mail Passport to Section,
    fill in Trip Explorer address.
  - Under Permanent Address Section,
    fill in Applicants address.
  - Do not sign this application until instructed by
    acceptance Agent.
• For those who have lost a valid passport, you need to also complete Lost Passport Form DS-64 .
• Proof of International Departure - Original flight itinerary
  from the airline or travel agent (Departure must be
  within 10 business days for those requesting a passport
  in less than 2 weeks).
• Check/Money Order of $135.00 payable to U.S. State Department & an Additional $25.00 to the Acceptance Agency. Do Not write checks until instructed to do so by Acceptance Agent (read INSTRUCTIONS below).
• Fill out & sign Trip Explorer Letter of Authorization.

For Trip Explorer

• Complete our Passport Order Form.
• Fill out & sign Trip Explorer Letter of Authorization.
• Proof of International Departure.
• Payment (Accepting Cash or Money Order from Post Office).

Note: Instructions

Take the above mentioned Required Documentation's of the Passport Office to an authorized Acceptance Agent (Post Office, Clerk of Court, Clerk of county), indicating that your passport application will be hand-delivered to the passport office. Ask them to execute the application and seal it in an envelope marked "TO BE OPENED BY PASSPORT OFFICE PERSONNEL ONLY". Do not leave sealed envelope with the clerk. You must present your original proof of photo identity to Acceptance Agent.

All applicant 13 years or younger must appear in person at the Acceptance Agent. Minor must be accompanied by both parent or legal guardian with acceptable proof of photo ID. Send paperwork plus sealed envelope to Trip Explorer for processing.

Once you received the Sealed Envelope from the Acceptance Agency, please call us before sending package with Payment to Trip Explorer.

Inside the package there should be:

• Sealed Envelope.
• Trip Explorer order form.
• Payment to Trip Explorer.

Note: Sealed envelope expires 5 business days after being sealed by clerk of court or postmaster.
